HOW DO I....
Register With The Practice?
If you live in our practice area and would like to register with
us, please complete one of our registration forms that are available from our reception team. They will
ensure that the necessary paperwork is fully completed. You will be registering with the practice rather
than an individual GP, though do please let the receptionist know if there is a particular practitioner you
would prefer to see.
Carers
You will be asked at your new patient medical if you are a carer
or if you yourself have a carer. Please give as much information as possible. Information, advice and support is
available for all carers.
HOW DO I....
Change My Personal Details?
If you change your name, address or telephone number please notify the surgery as soon as
possible. A change of address may mean you are outside the practice boundary. If you
are unsure please ask the receptionist who will advise you.
